Before you complete this form, do you have some images of yourself handy?  These do not have to be "professional" at all. Web cam stills or happy snaps are fine, as long as the images clearly show your face and full-length figure.  We enjoy working with amateur models!

Please only use JPEG (.jpg) files (not .bmp or .tiff) and don't attach massive files (500 pixels on the long side and 100 dpi would be ideal). If you don't have image resizing software, this application is fantastic and free. Unfortunately images can't be attached to this form so email them to us instead.
